     A very versatile script. Can set up multiple surveys, with multiple questions, and multiple answers within each question. Also allows users to select one option, or multi-choice options. You can also hide previous users's votes from the first screen. Once they have voted, shows number of votes and overall percentage of votes. Users can also leave comments about survey / site, (an admin section allows you to delete unwanted comments). **It's FREE for now, but may become part of a commercial package later.
